ISACA is committed to maintaining integrity and a level playing field for all examinees on any of ISACA’s tests. For these reasons, we have established this webpage containing both a Test Security Report Form so that our members, test takers, test administrators, other customers and vendors can report any test-related concerns in a confidential manner.

While it is extremely helpful to ISACA to have your name and contact information in the event we need your help with our investigation, you may choose to remain anonymous when you submit your question or concern. If you would prefer to report your concern over the phone, you can dial toll-free, within the United States, Guam, Puerto Rico and Canada: 855.207.1017. Otherwise, the most efficient and detailed method to provide all of the necessary information we need to investigate and remediate any fraudulent activity promptly is to complete the online form below. In completing the form below, one can also retain anonymity by entering only the required fields. If you desire a follow up or are willing to provide ISACA the opportunity to contact you for additional information, the form does allow you to provide your personal information.
